MINISTRY WORKERS INFORMATION FORM
Complete this form if you have recently joined or are interested in joining an ATM ministry/department. This form should be completed then given to the head of the ministry/department. The department head will submit to main office.
MULTIPURPOSE MEETING DETAILS FORM
